Dr. McDermott & Dr. Pyle would like to welcome you to Allergy, Asthma and Immunology of the Rockies.
The most important thing to us is to give you the best care and service we can. So you can plan ahead, we'd like to let you know that your first appointment with us could take up to 3 hours, depending on what's wrong with you and what tests and diagnoses are needed to help you. We also ask that you don't take any antihistamines for five to seven days before your appointment. This has to be done in order for the testing to give accurate results.
If you need to take an antihistamine, please do so and show up to your appointment on time. Then we'll decide how to move forward. Here is a list of medicines you should not take. You can get the forms you need for your first appointment from this page. You can find our HIPPA information in a separate document that you can only read, but you can print it out for your own records if you want to. Please carefully read all of the documents below, fill them out completely, and bring them with you to your appointment.
Either click on the Word doc to open the file and choose “save as” to save to your computer and print out a typed version. Type in your information, then print out the forms and return to the doctor’s office. For those who can’t open Word files, click on the pdf to print out and hand-write in your information and return to the doctor’s office. Please do not e-mail information for privacy. Please bring your insurance card and a photo ID to your appointment.
