New Patients



Dr. McDermott & Dr. Pyle would like to welcome you to Allergy, Asthma and Immunology of the Rockies. Our priority is to give you the best care and service possible. To plan accordingly, we would like to inform you that your first appointment with us could take up to 3 hours depending on your ailments and the testing and diagnostics needed to provide you with proper care. We also ask that you refrain from taking any anti-histamines for 5-7 day prior to your appointment. This is necessary to achieve accurate results from the testing performed.

If you find it necessary to take an anti-histamine, please do so and keep your appointment.  We will then decide how best to proceed.  Refer to the following list of medications to avoid. From this page you can download the forms needed for your first appointment. Our HIPPA information is available separately as a read only document, though you can print it for your own records if you wish. Please read all documents below carefully and fill them out completely and bring with you to your appointment.

Either click on the Word doc to open the file and choose “save as” to save to your computer and print out a typed version. Type in your information, then print out the forms and return to the doctor’s office. For those who can’t open Word files, click on the pdf to print out and hand-write in your information and return to the doctor’s office. Please do not e-mail information for privacy. Please bring your insurance card and a photo ID to your appointment.
